MassHealth NF-205: Three Changes Massachusetts Nursing Facilities Should Know
MassHealth Nursing Facility Bulletin 205, issued in November 2026 and effective October 1, 2026, updates several reimbursement and MDS audit requirements for Massachusetts nursing facilities.
For providers, three changes stand out, including more flexibility in respiratory assessment documentation, important limitations on behavioral add-on eligibility, and a simplified process for certain audit-related payment adjustments.
1. J1100C Respiratory Assessment Documentation Is Now Less Prescriptive
MassHealth has revised its documentation expectations for J1100C, which addresses shortness of breath while lying flat.
Under the previous bulletin, assessment documentation was expected to address specific questions, including whether the onset of symptoms was sudden or gradual.
NF-205 takes a less prescriptive approach. Rather than requiring documentation organized around specific questions, the updated guidance identifies relevant clinical factors that must be addressed, giving facilities greater flexibility in how they document their assessments.
However, that flexibility does not eliminate the need for supporting clinical evidence.
During the applicable seven-day look-back period, documentation must still support that the resident experienced difficulty breathing when attempting to lie flat or avoided lying flat because of breathing difficulty.
Relevant evidence may include clinician documentation, interviews, observations, medical history, symptom and positioning details, and an individualized care plan with measurable goals and interventions.
The takeaway: Facilities have more flexibility in how assessments are documented, but the clinical evidence and individualized care plan must still support the MDS coding.
2. MassHealth Behavioral Add-On Eligibility Depends on the Original MDS
This may be one of the most significant operational changes for Massachusetts nursing facilities.
Under NF-205, eligibility for the behavioral indicator add-on is determined using qualifying Section E scores on the original MDS assessment.
If a qualifying behavior was originally coded 0 or 1 and later corrected to 2 or 3, that correction cannot establish eligibility for the MassHealth add-on.
The affected items include E0200A, E0200B, E0200C, E0800, and E0900.
This reinforces the importance of capturing and documenting qualifying behaviors accurately during the original seven-day look-back period, including their frequency, interventions, and outcomes.
The takeaway: Facilities should review their processes for documenting behaviors and validating Section E coding before the original MDS is submitted.
3. MassHealth Audit Findings No Longer Automatically Require MDS Modifications
NF-205 also simplifies how certain audit-related payment adjustments are handled.
Facilities are no longer required to modify an MDS solely because of a MassHealth audit finding.
Instead, MassHealth may adjust the payment HIPPS code in MMIS based on the documentation supported during the audit.
Importantly, this does not eliminate MDS corrections required under CMS rules. Facilities must still distinguish between a MassHealth payment adjustment and a situation requiring an MDS correction.
The takeaway: Review audit response workflows to ensure staff understand when a payment adjustment is appropriate and when an MDS correction is still required.
What Should Massachusetts Nursing Facilities Review Now?
With these changes in effect, Massachusetts nursing facilities should consider:
- Reviewing respiratory assessment documentation and individualized care plans supporting J1100C.
- Validating Section E scores and supporting documentation before original MDS submission.
- Updating audit response procedures to distinguish payment adjustments from CMS-required MDS corrections.
- Reviewing pending audit cases and related MDS transmission or error reports.
While NF-205 provides greater flexibility in certain areas, it also reinforces the importance of accurate original assessments, consistent documentation, and well-defined internal processes.
